I'm a bit confused. This shows a Mains powered unit that produces a 12V output. It won't be of any use in the car.

With the Tom Tom you get an in-car charger with it - that's a cable which plugs into the Tom Tom and into the car cigar lighter socket and charges it up. The thingy you've seen is merely a converter plug which allows you to take it with you instead of the complete charging unit which also comes in the box - this thingy plugs into an electrical wall socket and has on the outside of it a socket just like the cigar lighter in the car. She would leave the charger cable attached when she brings the TT in from the car and simply plug it into this adapter thing in an ordinary electrical wall socket.

If all you want to do is charge the Tom Tom whilst in the car, all you need already comes with it - a cable to plug from TT to car cigar lighter socket.

This adapter thing you've found will NOT charge a phone from the car. For that you also need a phone in-car charger which works in the cigar lighter socket of the car. It'll cost you anything from £15 to £20 unless you can find a cheapo one somewhere - phones usually only come complete with a mains charger and you have to pay extra to get a car charger.
