The notes for this hot fix say it partially fixes a problem with background alerts, then recommends you don't update if you have the problem. As I have the problem should I update or not?

Depends on what your current version is. It's being discussed in this thread where it's recommended not to update if you have an older version than 1.3.6.

we have submitted another hotfix to Apple. It would be sinful to speculate how long it will take for them to approve it...
